It depends. My 15month old usually walks. I like for her to have as much independence as possible but occasionally she will go in the cart when either she is tired and cranky or not listening to Mommy and walking with me nicely (staying with me, holding my hand, not pulling stuff off of shelves etc)

Always in a carrier. No shopping carts or strollers.

It depends. I try and bring my stroller as much as I can because she can`t stand up like she tries to in the shopping cart. Plus I don`t have to worry about the germs even though I have a shopping cart cover.

at the grocery my LOs are always in the shopping cart. In any other store the small one is in the stroller and the big one walks with me.
