How is an episiotomy performed?

During the delivery of a baby, some women will require an episiotomy. An episiotomy occurs when the doctor takes a sterile pair of scissors and makes an incision in the tissue between the vagina and anus (the perineum).

Having an episiotomy was once considered to be a routine part of childbirth because doctors thought that a woman would heal faster from being cut than tearing naturally. With research and testing, doctors have now found that an episiotomy does not usually offer any great benefits. However, there are times that an episiotomy is still necessary during childbirth. Doctors will perform an episiotomy to make the vaginal opening larger if your baby needs to be delivered quickly. Doing so could prevent your baby from being stuck in the birth canal and suffering from oxygen deprivation. Your doctor may also perform an episiotomy if he feels that your vaginal tissues will tear extensively. A large vaginal tear may be more painful and take longer to heal than a simple episiotomy in this case.

An episiotomy is done in your labor/delivery room and is performed by your doctor. Your obstetrician will use a sterile pair of scissors to careful snip the perineum. If you have not had an epidural or pain medications, you will receive a local anestethic to numb the area to be cut. However, because of the pressure of the baby's head, you will not feel the cut anyways.

You can help soften the tissues in the vagina and perineum during the last few weeks of pregnancy by having perineal massage. Although the massage will not guarantee against tearing or needing an episiotomy, it may help to reduce the chances. You simply wash your hands well with soap and water. Using a lubricant, place your thumbs inside of the vagina and rub downwards towards the rectum. You should do this for 5-10 minutes each day. Your spouse or labor partner can help with the massage as well.

Healing from a vaginal tear or episiotomy takes some time. The perineum will be very sore for a few days to a few weeks after delivery, depending on the severity. Episiotomies generally hurt worse when you are sitting, walking or using the bathroom. It is critical that you keep the episiotomy site clean. Most hospitals will give you a squirt bottle to use for this purpose. Use an ice pack, witch hazel pads or a wound spray to help soothe the area. Take pain medications if needed and prescribed by your doctor. Be aware that when you are urinating, it may burn. Some women find that squirting warm water on their vagina while they are urinating helps to ease the burning sensation. Finally, take your time. Sit down carefully and walk slowly. Call your doctor if the wound becomes extremely painful, red, hot, swollen or has a foul odor or discharge.

I had my son 2 and a half weeks ago, and had an episiotomy and a fourth degree tear. I am still in pain, but I think I am healing pretty well considering. I am absolutely terrified of the thought of ever having sex again though. I know it's still too soon, but I can't help worrying. If anyone else who has had a fourth degree tear can send some reassurance, I'd appreciate it.

I had my son 8 days ago and am having a little bit of bleeding from my stitches when I have a bowel movement. The OB office said not to worry unless I see pus discharge or have an odor that would indicate infection, but I can't help but be scared I'm pulling something loose! My hubby's going to go buy a sitz bath for me to try today - any advice, ladies??

I had a third /superficial fourth degree tear/episiotomy with my 1st. I had to have an episiotomy with my 2nd b/c when they stitched me up the first time it was too tight. It probably took me about 2 weeks to recover.

my daughter was 8 1/2 lbs and i didn't get an episiotomy. My dr. took the time to crown the baby as she came out. I has the slightest tear which didn't even require a stich. Thank God. I was able to sit, drive and do everything I wanted right after.

i asked my dr about that perineal massage and he said dont waste your time, you wont know how much your bottom can actually stretch until that babies head is about to come out. He said that a lot of Midwifes may recommend it but in his experience he see's it as being a waste of time. Thats just what I was told.

I had my son 6 weeks and 2 days ago. I had an episiotomy then ended up tearing even more. Talk about one of the most painful parts of giving birth. I was open from one hole to another..12 stitches to close me up and I felt every single one. My husband and I just had sex for the first time the other night and it hurt like hell. We tried again the next night and about half way through it started to feel good. Don't give up if it hurts....it WILL get better!!

This is my 3rd pregnancy.. With my 1st I had a c-section. My 2nd I had a VBAC but pushed for 3 hours followed by an episotomy which ended up tearing more 4th degree. (my son had a HUGE head). I am so nervous this time- anyone out there that had 2 vaginal deliveries? Is the second one easier?? Thanks!!

to dresmommy.. congrats on actually wanting to have sex so soon!! you lucky lady!! i had a 10lb baby and was in quite a state down stairs to help me heal i used aromatherapy lavender drops in the bath once a day. its antiseptic as well as offers pain relief and as it is an oil help to keep the delicate skin supple... as other ladys have suggested maby massaging with a little almond oil will help the area. i had sex 5 weeks after my horric delivery i didnt really want to but i just wanted to see if it was ok. you sould like you must be well on your way to healing already!!! xxx

With my first one I tore a little (just a couple of stitches), but with my second my midwife taught me how to push without tearing. As soon as I feel a burning sensation, I let off of the push until it quits burning--not matter what I'm told to do! It worked great with the second and third. I didn't tear at all!

w/ my son the dr let me tear naturally - he only weighed 6lbs 11oz and i had a 4th degree tear. it was a rough recovery. i had my little girl 2 weeks ago and when she was crowning my dr (diff one) kept saying that she was such a tight fit and asked if he could cut me. he gave me a 2nd degree episiotomy and i was only sore for about 4 days. i had no trouble going to the bathroom (even immediately after birth). i think it just depends on the degree of your tear/cut.

Hello all. I just wanted to give you my pointof view on episiotomy vs. natural tear. With my first son (7lbs 14oz) I had a male md during delivery and I had an episiotomy. It was painful for awhile. I even took the sitz bath home with me and used it for a few days. I felt like I could feel it everytime I moved. I constantly used those tucks pads because they have a cooling soothing effect. They are normally used for hemmoroids but they really worked for my incision. With my 2nd son (8lbs 61/2oz) I had a woman md during delivery. During birth I had a small tear that they stitched with a few stitches. The healing time was much faster. I did not even really feel the soreness much after I went home 2 days later. When we were packing bags for the hospital I made sure I had those tucks pads with me. I di not even open them after I had my 2nd son. Maybe you can discuss with your doc your options ahead of time. If you may be better off if you slightly tear naturally compared to being cut. This was just my experience and everyone is different. During my next birth I will ask them not to cut me unless I am at risk for a large tear or they need to get the baby out quickly.

With all three of the births of my sons, i didnt have an episiotomy. I did however tear just a little which only required 1-2 stiches. I honeslty feel lucky thaty my dr.s didnt go the route of the episotomy. Basically, when the dr. is instructing you with when to push and when to stop and you listen, this lessens the chance of tearing. Also the dr. or midwife, can massage the perineum as you are pushing and this helps with it relaxing and allowing the head to come through easier and with less tearing. This is how it went with me. I am expecting that my fourth pregnancy should go pretty much the same way :). No episiotmy for me thank you.

well i had my son 2 weeks ago (feb 19th)) and got an episiotomy and ended up tearing a little...it was horrrrible for the next few days but now it really doesnt hurt at all...my problem is...at the end of this month (march 21st) my man and me are planning a trip to the beach and its really gonna stink if were not able to have sex...does anyone know of anything to help the area heal faster or do you think i should be okay by then? how long does it take the stitches so dissolve? and alsooo i was wondering...the nurse in recovery told me not to take a bath for awhile because my cervis was still open...but ive heard other people say they took one right after delivery...how long should i wait? thankkk you.

I had episiotomy done during my labor. It has been six weeks and I was wondering why my butt still hurts at times when I sit. I went back to my oby/gyn and she told me she sewed on the skin of my vagina by mistake, so I had to wait to heal for another two weeks. It is very painful to have sex, and now I kind of have a phobia when my husband asked me for sex. After more than two months, I had the courage to touch the stitches part and my butt, I discovered that there is a peanut-size thing sticking at the perineum. I asked my doctor and she said it is hemorrhoids and that the hemorrhoids will always be there. I knew that my hemorrhoids already healed, and I knew that this peanut-size flesh is not part of the anus, and is not hemorrhoids, so I went to my provider, and have her inspect it, she said I still have some inflammation on the part that got stitched, and that the peanut size flesh is a submucosa. When the oby/gyn doctor stitched me up, that part was kind of like a left over piece. That part has been tender, and sensitive, and I really do not know if I need a get a second opinion and have that part fixed. I am so afraid of surgery, but that submucosa sticking out really worries me a lot. I don't know if that submucosa is part of what is causing the pain during sex. I am still observing and will go back to my provider after one month. I wonder if I should have asked for a caesarean so I did not have to deal with all these things - the oby/gyn screwing up with the stithces, hemorrhoids, and that submucosa sticking out from the perineum. I wonder if there is anybody who has the same experience as I have. I wanted to have a second child, but now I am so afraid to get pregnant again because of this experience.

Even though I had an episiotomy w/ my daughter, I am terrified of having one again with this one. I'm sure I will need it b/c my daughter only weighed 4 lbs. 14 oz (was 6 weeks premature) and so if this one goes even just a little bit farther he will surely be bigger than she was... the episiotomy for me was the worst part of the entire labor/delivery and I am not looking forward to it, but if i have to have one I guess there is nothing I can do about it, just suck it up and deal with it....

srm31 - This is my second pregnancy and I had an epidural and an episiotomy with my son. I pushed for about an hour and 15 minutes and then once they gave me an episiotomy he came after two or thre pushes. I am definitely going to ask them for an episiotomy this time if it appears that it will be difficult to deliver my daughter. WIth the epidural I didn't feel the stiches afterwards. I just took the motrin that the hospital gave me and the pain after the epidural was really manageable. Good Luck!

I'm from the day when they always did one. Now it seems more frowned upon. My last birth I had one and I still tore ( but on my labia ...not where the episiotomy was ) because my daughters head was so big. So now I don't know which way to go. Doesn't it hurt more to tear than to just get one in the first place ? I felt that burning tear more than the cut. I didnt have any medication either so I'm considering an epidural this time around as well. Thoughts ?

my episiotomy still hurts..and its 2 weeks and 3 days later....it doesn't hurt so much when im sitting still, but as soon as i move the pain comes on....its annoying when im trying to get around and breastfeed and take care of my baby boy. they seem to be healing pretty well i got cut all the way to my anus, sooo that was no very plesant...i have about 1 lose stitch that caused me alot of pain...i am just hoping that in a few weeks it will all be healed up because labor wasn't even this bad :(. if i had a c section my pains would be less than they are now..i just hope none of you hurt as bad as i have. !!

Hi! Just a tip... Make sure you keep an eye on the area if you do need this procedure done, meaning get a hand held mirror to look! I made the mistake of not checking the area then on Friday, 4 weeks after having my baby, I discovered that my stitches had torn right open and my wound was a huge open hole, I was almost able to see my bladder. I had to go to the doctors who had to re open the part of the wound that healed which was healing crooked and they had to give me more internal and external stitches! Not a very pleasant thing to experience...

I delivered a 9.7 lb baby vaginally, she didnt want to come out so the doc give me a local anastetic shot, I had no epidural, then he cut me just a little then baby come out so much easier.The 3-4 days after birth were the worse, then gradually got better. Always keep the water squirt bottle handy while using the bathroom and a towell or good toilet paper to dab the area after rinse. I used an antiseptic/pain relief spray like solorcain everytime i went to the bathroom.. I was so busy with trying to get baby tended to and the breastfeeding I sometimes forgot about the stitches,,, until I had to move LOL.. also ask doctor about a stool softener, #2 is too painful to even attempt to push at all
